| Full payment of your course fee must be provided at the time of registration. Payment can be made by check (payable to Productivity by RFID, mailing address above), American Express, MasterCard or Visa. |

| ? Cancellations/Substitutions |

| If you must cancel, you should send a written notice no later than 30 days prior to the workshop for which you've registered. All monies will be refunded, less a $100 cancellation fee. If you cancel less than 30 days prior to the course, but before 10 days prior to the course, monies will be refunded less a 50% cancellation fee. No refunds will be given if you cancel 10 days or less prior to your scheduled workshop. Substitutions will be accepted only with the written permission of the original registrant and with the approval of the workshop producer. All cancellations and substitutions must be received in writing and emailed to info@pbrfid.com or faxed to (216) 751-1154. |

| Space at The Mechanics of RFID workshop is limited. The Mechanics of RFID workshop is specifically designed for users and potential users of RFID technologies. Acceptance of registration from systems integrators, consultants, and analysts will be made on a case-by-case basis. The workshop is not appropriate for non-sponsoring technology vendors. All registrations are subject to approval by the workshop producers. Your e-mail confirmation note will serve as your acceptance to the course. Any monies collected from non-accepted registrants will be refunded within three business days. |
